ABSTRACT

Abstract The aim of this study was to describe the prevalence, clinicopathological, and prognostic features of acinic cell carcinoma (AciCC) of the oral and maxillofacial region. AciCC cases were retrospectively retrieved from 11 pathology centers of three different countries. Medical records were examined to extract demographic, clinical, pathologic, and follow-up information. A total of 75 cases were included. Females (65.33%) with a mean age of 45.51 years were mostly affected. The lesions usually presented as an asymptomatic (64.28%) nodule (95.66%) in the parotid gland (70.68%). The association of two histopathological patterns was the most common finding (48.93%) and the tumors presented mainly conventional histopathological grades (86.11%). Surgical treatment was performed in the majority of the cases (59.19%). Local recurrence was observed in 20% of the informed cases, regional metastasis in 30.43%, and distant metastasis in 12.50%. The statistical analysis showed that the cases with a solid histopathological pattern (p=0.01), high-grade transformation (p=0.008), recurrence (p=0.007), and regional metastasis (p=0.03) were associated with poor survival. In conclusion, high histopathological transformation, presence of nodal metastasis, and recurrence were prognostic factors for AciCC of the oral and maxillofacial region.

Resumo Objetivo analisar a lotação e distribuição da primeira turma de oficiais enfermeiros, ao término do Curso de Formação de Oficiais, com a patente de 2º Tenente, no Hospital Central da Polícia Militar do Estado do Rio de Janeiro, em 1995. Método estudo histórico em fontes documentais escritas e orais produzidas por meio de 19 entrevistas, sendo 18 oficiais enfermeiros e um civil. Os conceitos de poder simbólico e capital de Pierre Bourdieu consubstanciaram a análise dos achados. Resultados a classificação obtida no curso de formação de oficiais e a experiência profissional, embora com menor peso, foram preponderantes na distribuição nos setores do hospital. Considerações finais e implicações para prática os critérios de antiguidade da vida militar influenciaram na configuração da equipe de enfermagem no hospital militar. Destaca-se que a ocupação das chefias pelos oficiais enfermeiros foi determinada pelo capital incorporado no estágio probatório, o qual foi institucionalizado pela patente.

Método estudio histórico en fuentes de documentos escritos y orales producidos a través de 19 entrevistas, 18 de las cuales fueron enfermeras y una civil. Los conceptos de poder simbólico y capital de Pierre Bourdieu fundamentaron el análisis de los hallazgos. Resultados la clasificación obtenida en el curso de formación para oficiales y la experiencia profesional predominaron en la distribución en los sectores hospitalarios, aunque con menor peso. Conclusión e implicaciones para práctica los criterios de antigüedad en la vida militar influyeron en la configuración del equipo de enfermería. Se destaca que la ocupación de los comandos por oficiales enfermeros fue determinada por el capital incorporado en la etapa probatoria, institucionalizada por el grado.


Abstract Objective to analyze the capacity and distribution of the first class of nurse officers, at the end of the Officer Training Course, with the rank of 2nd Lieutenant, at the Hospital Central da Polícia Militar of the State of Rio de Janeiro, in 1995. Method historical study in written and oral documentary sources produced through 19 interviews, of which 18 are nurse officers and one civilian. Pierre Bourdieu's concepts of symbolic power and capital substantiated the analysis of the findings. Results the classification obtained in the training course for officers and professional experience, although with less weight, were preponderant for the distribution in the hospital sectors. Final considerations and implications for practice the criteria of seniority in military life influenced the configuration of the nursing team at the military hospital. It is noteworthy that the occupation of the heads by the nurse officers was determined by the capital incorporated in the probationary period, which was institutionalized by the patent.

ABSTRACT

Objetivo analisar a evolução histórica da configuração da equipe de enfermagem em um hospital militar. Métodos estudo histórico-social. Fontes históricas: documentos escritos e depoimentos orais cedidos por cinco enfermeiras militares. Os dados coletados foram analisados em conformidade com o método histórico, após organização e classificação. Resultados na chegada dos oficiais enfermeiros ao Hospital Central da Polícia Militar, a equipe de enfermeiros estava reduzida a quatro enfermeiros civis e à enfermeira-chefe. Contava também com cento e setenta sargentos e cabos de saúde e trezentos e quatro funcionários civis (técnicos e auxiliares de enfermagem). O cuidado de enfermagem era majoritariamente realizado por profissionais de nível médio que já pertenciam à corporação. Conclusão os oficiais enfermeiros enfrentaram sobrecarga de trabalho, além das dificuldades com "os praças" (militares das categorias inferiores), no tocante ao acatamento da hierarquia, ocorrendo alguns embates, ensejados por disputas de poder, em um espaço misógino.


Objective to analyze the historical evolution of configuration of the nursing team in a military hospital. Methods a social-historical study. Historical sources: Written documents and oral statements given by five military nurses. The collected data were analyzed according to the historical method, after organization and classification. Results when nurse officers arrive to the Military Police Central Hospital, the team of nurses counted with four civilian nurses and the chief nurse. It had 170 health sergeants and soldiers and 304 civil servants (nursing technicians and auxiliaries). Medium-level professionals who already belonged to the corporation mostly performed nursing care. Conclusion the nurse officers faced work overload, in addition to the difficulties with the military staff of lower categories, regarding the compliance of the hierarchy that caused some conflicts, because of power disputes, in a misogynist environment.
